ACCEPTABLE PROOF OF INCOME:

• For Employed- Certificate of Employment and Payslip indicating your salary.
• For Self Employed- Mayor’s Business Permit or Barangay Business Permit or Picture of Business with Declaration of Income.
• For Recipient of Remittance- 3 months latest remittance slip or SOA/bank statement
• For OFW- Latest Contract
• Pensioner- Bank Statement/ Statement of Account
